The eForms listed below are replicas of the forms available in the church office. They can be completed and submitted to the church electronically. Some forms are password protected. To obtain the password, contact Sis. Juanita Washington at the church office (215 242-5550).
Click on the form’s title to open an eForm. Once you have electronically completed the form, send it to the church by pressing the “Submit by Email” button located at the bottom of the form. The form will be attached to your email and ready to be sent to the business/church office.
If you are unable to submit the form successfully, please save the file to your hard drive: select File, Save As, name the document. Open the document from the saved location and email it to firstname.lastname@example.org.
